Open Google Documents page

Create a new Google Form

Name the form

There are 2 sample questions in each new form. Click on the pencil icon to edit the question.

Edit the question type.

Write test questions using a variety of question types

Share form with target audience. Click on "Email the Form" and type in email addresses. Addresses will be saved if re-sending is necessary.
